NAV-CARE IS AVAILABLE TO ALL ADULTS WITH DECLINING HEALTH OR EXPERIENCING A QUALITY OF LIFE CONCERN LISTED IN THE FIRST QUESTION. THE COMMUNITY CONNECTOR PROGRAM IS ONLY AVAILABLE TO ADULTS AGED 55+.

There is no need to have a specific diagnosis or condition; what is important is that the person accessing the service feels that they can benefit from being matched with a volunteer. You can refer yourself or be referred by a friend, family member, medical professional or concerned community member. We will always ask if they have your permission before we contact you.
